Program of All-Inclusive Care for the Elderly – is a non-profit health care program created to help people over the age of 55 in Onondaga County live safely while receiving the care they need. It is particularly helpful to families desiring to keep their aging parents and loved ones at home.
Participants receive home care, medical, social, rehabilitative, and other support services needed at two PACE centers (one in Syracuse and one in North Syracuse), and their own place of residence. PACE CNY is an alternative to nursing home care, and is sponsored by Loretto in cooperation with St. Joseph’s Hospital Health Center.
PACE CNY serves Onondaga County residents age 55 and over from all income levels, including those who are Medicaid eligible. PACE CNY is a long-term managed care program. PACE CNY participants may be fully and personally liable for the costs of services not authorized through the PACE CNY program (other than emergency services).
